RESEARCH AND AUDIT
RESEARCH AND AUDIT
Research is a valued part of the departmental activities. Several research
and audit projects are currently being undertaken in the department of
Neurosurgery at Frenchay Hospital. These include laboratory-based scientific
studies, clinical research into neurosurgical disorders, clinical trials, and
audit projects which are aimed at improving the standards of patient care in our
unit.
There are two principle research units within our department: the Neurosurgery
Research Group and the Spinal Research and Development Unit.
- Neurosurgery Research Group
The particular interests of the group include head injury, subarachnoid
haemorrhage, trigeminal neuralgia, acoustic neuromas, pituitary tumours
and functional stereotactic surgery for Parkinson's disease and movement
disorders.
- Spinal Research and Development Unit
In collaboration with Medtronic Sofamor-Danek this group is involved with
the investigation of spinal instrumentation in the treatment of
degenerative spinal disorders, image guidance in spinal surgery, and the
outcome of surgery for cervical spondylosis.

- The neurotrophin and glucocorticoid responses to traumatic brain injury
in relation to outcome. Mr P Grundy, Dr P Sharples, Prof S Lightman
- The biomechanics of the cervical spine. Crispin Wigfield, Richard
Nelson (Department of Anatomy, University of Bristol)
Major Areas of Clinical Research
- The surgical treatment of Parkinson's Disease - Subthalamic Nucleotomy & Growth factor treatment in human Parkinson's disease. Mr Nik Patel, Mr S Gill
- Application of image guidance to the surgical treatment of spinal
disorders. Mr C Wigfield, Mr C Bolger.
- The use of an artificial cervical joint in the treatment of cervical
spondylosis. Mr S Gill, Mr R Nelson, Prof. J T Robertson, Mr C Wigfield.
- Interbody fusion devices in the surgical treatment of cervical
spondylosis. Mr C Wigfield, Prof. J T Robertson, Mr R Nelson.
- Prospective outcome of single level anterior cervical decompression
and fusion for the treatment of cervical spondylosis. Mr C Wigfield,Prof J T Robertson, Mr R Nelson.

CLINICAL AUDIT
- Audit of the process and documentation of informed consent
Mr R Nelson, Clinical Standards Committee, Department of Neurosurgery
- Functional hearing preservation following acoustic neuroma surgery Prof H Coakham, Mr R Edwards
- Outcome of Acoustic Neuroma surgery Prof H Coakham, Mr R Nelson,
Y Clarke.
- Audit of Microvascular Decompression for Trigeminal Neuralgia, Acoustic Neuroma & Endoscopic Surgery Professor H Coakham, Y Clarke
Ongoing database of patients treated since 1991. Range of patient follow-up 1-19 years (mean 6.25 years).

- Diagnosis and surgical management and Outcome of Arachnoid Webs causing trigeminal neuralgia, and outcome following surgery Prof. H Coakham, Dr S Renowden
n= 5, completion Feb 2001
- Vascular Audits Mr.R Nelson, Dr S Renowden, Dr D Ainsley-Williams
Subarachnoid haemorrhage: Ongoing database of patients since 1995. Writing of research papers and presentations of findings nationally and internationally
Arterio-venous malformation: Currently setting up a database
Outcomes following balloon occlusion of Carotid Artery
- Pituitary Surgery Mr D Sandeman, Mr R Nelson
Baseline data for patients 1993-1998. Ongoing database of patients admitted since August1998
- Surgical outcomes following selective temporal lobectomy for temporal lobe epilepsy Mr David Sandeman
n=60, with 5-15 new patients per year. Multi-professional audit: Neurosurgery, Neurology, Neurophysiology, Neuropsychiatry, Neuropsychology)
- Vagal Nerve Stimulators for Epilepsy Mr D Sandeman
Outcome of vagus nerve stimulator implants.n=40, with 20 new patients per year. Multi-professional audit

n=95pts, Completion Feb01
- Shunt Infections in patients with Ventriculoperitoneal shunts Mr I Pople
All shunt procedures are entered into the Department of
Health-backed UK Shunt Registry. Our shunt infection rate
for children under 5 years of age is one of the lowest in the country (c.1%).
Our overall shunt infection rate has been reduced from 10% to 2.5% by
implementation of a Shunt Protocol five years ago. This annual audit is benchmarked nationally.
- Endoscopic ventriculostomy in the treatment of intracranial
tumours. Prof H Coakham, Mr I Pople, Y Clarke
A recent audit of endoscopic procedures at Frenchay Hospital showed a
success rate of 75% (international average: 65-75%) and a complication
rate of 8% (infection and minor haemorrhage).
